Survey boxes...

This site has the thing that I want (it's my dads site, so no worries!). I want to do like this, but I'm using registercheaper.com. How do i do those boxes??? And how to I get it so that when they hit the submit button, it sends the form to my email???

I think you want an e-mail form. Her's what I use:http://www.response-o-matic.com It goes directly into yo're e-mail account. You can edit it and Take out things. It dosen't give you're visitors a message either
